You may not know the answer to some of the questions, feel free to leave those blank and they will be discussed at your next meeting with your consultant. We will review your answers prior to that meeting and be fully prepared.

Please be aware that the security of the application means that you will only be able to log in from the same location (IP address) each time, and in addition if you do not update the page for 20 minutes you will automatically be logged out.

There is no need to include any personal data within your answers, in order to maintain alignment with GDPR we would ask that you avoid doing so.

Please be aware that completion of these questions, not matter how comprehensively, will NOT make you aligned with the new data protection regulation, this is simply the information from which we build a picture in order to provide you with the tools for that end gain.
